I have a similar problem. I previously submitted the paper form and had it all sorted. Then the website changed and showed no beneficiaries. I tried to input the information on the new site but couldn't because my spouses SSN was already in use. They have their own TSP also. I called and the rep told me to submit it without the SSN. I want to use the SSN because I have an ex-spouse with a very similar name and want to be very clear who gets what. The TSP rep says that if I submitted the form previously that we should be all good. I say great- who does the TSP show as my beneficiary? They couldn't tell me, didn't have that information. And now you can't even find the TSP-40 form and if you did it probably wouldn't be accepted.
